RenalConnect

RenalConnect is a secure, open-source, and web-based app that helps clinicians manage end stage renal disease (ESRD) patients who require some form of dialysis or transplant. About two years ago, this app was launched in several hospitals affiliated with the largest health authority in British Columbia, including Royal Columbian Hospital, Surrey Memorial Hospital, and Abbotsford Regional Hospital. Since then, the app has helped manage over 200 patients on dialysis and was instrumental in reducing the peritonitis rate at the renal program from 1 in 24.4 months to 1 in 36.0 months, a 47.5% reduction in frequency. The results of this project were presented at the ASN Kidney Week in October of 2012.
